Qualifications

• Requires LPN/LVN, LSW, LCSW or LMSW license per state law.

• Current, unrestricted licensure as applicable in state(s) required.

Responsibilities

• Identify members for high risk complications and obtain clinical data.
• Identify members for alternative level of care or waiver programs.
• Collaborate with RN to develop and monitor care plans.
