Job summary

Elevance Health is seeking an LTSS Service Coordinator-Clinician to support members with physical health, behavioral health, long term services, and social needs. This field-based role involves traveling to client sites and occasional meetings, with hybrid/onsite expectations per policy.

The Clinician will conduct telephonic or face-to-face assessments under licensure, coordinate care, and participate in care planning to address complex member needs while ensuring efficient use of benefits.

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or licensing as allowed by state law and Nursing Diploma or AS in Nursing or related field with minimum 2 years of experience with chronic illnesses, comorbidities, or disabilities in a service coordination or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license (as allowed by state law) in applicable state(s) required.

Responsibilities

• Assists in identifying members for high risk complications. Obtains clinical data.
• Assists in identifying members that would benefit from an alternative level of care or other waiver programs.
• Provides information collected to RN, who verifies and interprets the information, conducts additional assessments, and develops, monitors, and revises the member's care plan.
• Participates in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities with the RN and health care team to ensure cost-effective utilization of health benefits.
• Decision making based on member needs, understanding disease processes and clinical guidelines; nursing judgment not required.
