Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician to oversee member care plans across health and social services. The role involves telephonic or face-to-face assessments, developing and monitoring care plans, and coordinating with non-RN clinicians under state law and contract requirements.

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or Case Management role.
• Current, active, valid RN license in applicable state(s).
• May require state-specified certification based on state law and/or contract.

Responsibilities

• Identify members for high-risk complications and coordinate care with the health team.
• Manage members with chronic conditions to ensure cost-effective, appropriate use of benefits.
• Obtain thorough member history to develop individualized care plans.
• Set short and long-term goals with the member, caregivers, and physicians.
• Determine if a member may need an alternative level of care or other waiver programs.
