Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to support field-based member care. You will work under an RN, perform telephonic or face-to-face assessments, and coordinate with the health team to identify risks and plan long-term services.

The role requires licensure in applicable state law, minimum 2 years' experience with chronic illnesses, and flexibility to travel to member sites within designated counties.

Qualifications

• LPN/LVN, LSW, LCSW, or LMSW or license other than RN as allowed by state law.
• Minimum of 2 years with individuals having chronic illnesses/disabilities in a service coordination role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or equivalent license required by state law.
• State-specific certification may be required by contract.

Responsibilities

• Assist the RN in identifying high-risk members and data collection.
• Support development and modification of care plans with the health team.
• Coordinate care for members with chronic illnesses and disabilities.
• Provide information to the RN for assessments and care planning.
• Help ensure cost-effective, appropriate utilization of health benefits.
• Apply clinical guidelines while leveraging disease knowledge without nursing judgment.
