Qualifications

• Requires an LPN/LVN, LSW, LCSW, LMSW or RN-allowed license with 2+ years of chronic illness experience.
• Current uninterrupted license in applicable state(s) is required.
• Preferred case management experience with older adults or individuals with disabilities.

Responsibilities

• Assists in identifying high-risk members and obtains clinical data.
• Identifies members who may benefit from alternative care or waivers.
• Supports RN in developing, monitoring, and revising the member's care plan to meet needs.
• Coordinates care for members with chronic illnesses to ensure cost-effective and efficient use of health benefits.
• Applies clinical guidelines in decision making without requiring nursing judgment.
