Centene is seeking a field-based Care Manager to develop and coordinate holistic care management activities, including in-home visits and education for long-term care members. The role supports individuals enrolled in PD/Elderly Waiver programs and requires approximately 50% local travel with remote work for the rest.

• Requires a Bachelor's degree and 1 year of related experience or equivalent experience.
• For Illinois Plan Only: meet one of the 3 criteria: RN licensed in Illinois; Bachelor/Master’s in human services field; or LPN with 1 year experience.

Responsabilidades

• Evaluates the needs of the member, resources available, and recommends the best outcome.
• Assists with developing long-term care plans and identifies providers and community resources.
• Coordinates between member/family and care provider team to ensure services are accessible.
• Provides resource support to members and families for various needs.
• Monitors care plans/services and outcomes, and provides recommendations.
• Interacts with long-term care providers and partners to meet member needs.
• Collects and maintains member information to ensure regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
• Provides education to members and families on procedures, options, referrals, and benefits.
• Provides leadership with opportunities to improve quality and cost-effective care.
• Performs other duties as assigned and complies with policies.
