Job summary

Humana, Inc. seeks a Transition/Custodial Prevention Specialist to coordinate long-term services and supports for members in Illinois. You will meet members in the field, assess goals and barriers, and connect them with community resources to promote better health outcomes and independence.

You will travel regionally (75-90%) and work a 40-hour week with some onsite meetings in Schaumburg, IL. A bachelor's degree and Medicare/Medicaid experience are required; IL credentials such as LPC/LSW/RN

Qualifications

  • Must reside in the state of Illinois.
  • Bachelor's degree in social sciences, social work, human services, or a related field.
  • Two years of experience with Medicare/Medicaid recipients or HCBS.
  • One year or more experience with custodial and transitional care.
  • Experience using electronic information applications, including EMR.

Responsibilities

  • Meet members in the field to assess goals, needs and barriers.
  • Coordinate services and develop plans of care for LTSS.
  • Connect members with LTSS providers and community resources.
  • Support integration of care teams and navigate HCBS needs.
  • Educate members to maintain Medicaid eligibility and use resources.
