Job summary

Pacific Health Group is seeking a field-based housing stability specialist to support CalAIM Community Supports, housing navigation, and case management for high-needs members. You will spend most of your time in the community conducting outreach, housing assessments, and coordinating with landlords and providers.

You will build strong community partnerships, document care plans in real time, and ensure timely housing placements while navigating multiple systems and maintaining high professional

Qualifications

• Strong understanding of housing systems and local resources
• Experience with Medi-Cal, CalAIM, and Community Supports (preferred)
• Excellent communication, empathy, and cultural competence
• Proficient with case management systems and real-time documentation

Responsibilities

• Assist with Housing Transition Navigation and Housing Tenancy & Sustaining Services
• Coordinate housing deposits, navigate deposits and landlord coordination
• Field-based member outreach with home visits, assessments and follow-ups
• Build partnerships with housing providers, shelters and community organizations
• Document in real-time and maintain complete records per state guidelines
