Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support CalAIM Community Supports housing programs in San Joaquin County, including Stockton and Lodi. You will conduct intake assessments, coordinate housing deposits, and partner with landlords and shelters to secure long-term housing.

This role requires independence, strong time management, and the ability to travel locally across the county.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports preferred.
• Proficient documentation and real-time data entry.

Responsibilities

• Housing stability services and housing navigation for eligible members.
• Coordinate housing tenancy, deposits, and long-term housing support.
• Conduct field visits and community outreach across San Joaquin County.
• Document in real-time and ensure compliance with state requirements.
• Engage with housing providers, shelters, and community partners.
