Job summary

Pacific Health Group in California is hiring for a field-based Housing Stability Services professional. You will support CalAIM Community Supports Housing programs, conduct outreach, and coordinate housing resources with local providers to help members achieve stable housing.

The role emphasizes independent field work and timely documentation, with collaboration across health and social services to ensure housing is a driver of health. Hybrid work options available.

Qualifications

• Strong understanding of housing system and local housing resources
• Experience with Medi-Cal, CalAIM, and Community Supports is preferred
• Excellent communication, empathy, and cultural competence

Responsibilities

• Provide Housing Transition Navigation and Housing Tenancy & Sustaining Services
• Assist with Housing Deposits and deposit documentation
• Coordinate with housing authorities, landlords, shelters and providers
• Conduct member assessments and develop housing care plans
• Work approx. 50% in the field and travel locally
• Perform in-person visits, assessments, and real-time documentation
• Engage in community outreach and referrals
• Manage application pipeline for housing resources
• Collaborate with interdisciplinary teams
• Ensure Medi-Cal/CalAIM/Community Supports compliance
