Job summary

The People Concern, in collaboration with the City of Los Angeles, seeks a SUNOFO Housing Navigator to support a fast-paced housing-focused program. The role bridges field work with interim housing site partnerships, handling housing applications and coordinating with housing authorities, PSH providers, and landlords for program-enrolled clients.

Qualifications

• Bachelor's degree in related field (Social Work, Community Organizing, or other Human Services) and one year of full-time experience with people experiencing homelessness.
• Knowledge of homeless social service providers and Coordinated Entry System.
• Able to communicate with subcontractors, housing providers and community partners in a culturally competent manner.

Responsibilities

• Carry a case load of 20-25 clients to prepare housing interventions.
• Develop housing plan in collaboration with clients detailing needs.
• Complete HMIS documentation within one month in partnership with IHP staff.
• Assist clients with obtaining identification and Social Security card, disability verification, homelessness verification, and other documents for permanent housing.
• Serve as the client's point of contact for permanent housing resource matching.
• Notify treatment providers when a client is matched to a permanent housing resource.
• Complete housing applications, including the Universal Housing Application within seven days of invitation.
• Locate available permanent housing units and transport clients to view units.
• Identify housing resources and build relationships with landlords to increase permanent housing opportunities.

Education
