Job summary

The People Concern in Santa Monica is seeking a SUNOFO Housing Navigator to support housing-focused services for people experiencing homelessness. The role operates both in the field and at Interim Housing sites, coordinating housing applications, transporting clients, and connecting them with permanent housing resources.

You will manage 20-25 client cases, collaborate with City partners and housing providers, document progress in HMIS, and ensure leases, eligibility, and move-in tasks are

Qualifications

• High School Diploma or equivalent is required.
• Bachelor’s degree in Social Work, Community Organizing, or related field preferred.
• Knowledge of homeless service providers and Coordinated Entry System.
• Able to communicate with partners in a culturally competent manner.
• Detail oriented with good time management, written and verbal skills.
• Self-starter who thrives in a fast-paced environment.
• Ability to collaborate with a wide range of stakeholders.

Responsibilities

• Carry a caseload of 20-25 clients to support successful housing interventions.
• Develop housing plans in collaboration with clients.
• Complete HMIS documentation within required timelines.
• Assist with identification, verification, and housing application processing.
• Serve as point of contact for permanent housing resource matching.
• Coordinate client transportation to view and apply for housing.
• Review leases and help clients understand tenancy requirements.
