Job summary

National Health Foundation in Los Angeles (Pico Union) is seeking a Housing Navigator to provide direct support to participants facing housing crises and homelessness. You will engage clients, perform intensive case management, and coordinate with housing authorities and service partners to move individuals toward permanent housing.

The role requires collaboration with interdisciplinary teams, experience in housing programs, and a commitment to ending homelessness.

Qualifications

• BA degree preferred in a social services or related field.
• Associate degree preferred; minimum of three years of experience in social services.

Responsibilities

• Conduct initial meeting and assessment to build rapport and evaluate housing needs.
• Review intake and assessments for guests to evaluate housing needs and service requirements.
• Connect participants to community resources supporting permanent housing (benefits advocacy, housing programs).
• Develop Individual Action Plans and assist guests in accessing services addressing needs and goals.
• Provide housing stabilization services (money management, lease understanding, life skills, transportation).
• Establish relationships with landlords and housing providers to expand housing options.
• Enter data in required databases and maintain accurate guest files.
