VitalCaring in Waxahachie, Texas is seeking a Care Transition Coordinator to connect patients, families, and healthcare partners to hospice services. This field-based role serves as the primary liaison between referral sources and internal clinical teams, ensuring seamless, timely transitions of care.

You will conduct strategic outreach, nurture referral partnerships, manage CRM/EMR documentation, and travel within the territory to support admissions and patient access to care.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field; or equivalent experience.
• Minimum of two years of healthcare or related sales experience.
• Strong communication and relationship-building skills.
• Ability to manage multiple priorities in a fast-paced, field-based environment.
• Proficiency with Microsoft Office and CRM systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured outreach calls, develop relationships, identify needs, and drive referrals.
• Build and maintain relationships with referral partners through consistent communication and service excellence.
• Develop strategies to grow market share and drive referral volume.
• Track and document sales activity across referral sources using CRM tools.
• Support timely patient admissions with cross-functional coordination.
