Job summary

VitalCaring is seeking a Care Transition Coordinator to connect patients, families, and healthcare partners with hospice services. This field-based role blends healthcare sales, care coordination, and partnership development to drive patient access to care.

You will serve as the primary liaison between referral sources and internal teams, ensuring timely, seamless transitions and growth in referrals while supporting outcomes and organizational goals.

Qualifications

• Bachelor's degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two years of healthcare or related sales experience.
• Proficiency with Microsoft Office and CRM systems.
• Strong communication, relationship-building, and influencing skills.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Build and maintain strong relationships with referral partners through consistent communication and service excellence.
• Develop and execute strategies to grow market share and drive referral volume.
• Track, manage, and document sales activity across referral sources using CRM tools.
• Achieve established referral and admission goals.
• Establish trusted partnerships with hospitals, physicians, case managers, and community stakeholders.
• Collaborate closely with clinical and operational teams to support timely and appropriate patient admissions.
• Represent VitalCaring professionally across healthcare settings and participate in community outreach.
