VitalCaring Group seeks a Care Transition Coordinator to connect patients, families, and healthcare partners for seamless transitions into home health or hospice services. This field-based role emphasizes proactive relationship-building, CRM usage, and timely admissions.

The position blends healthcare sales with care coordination to support patient outcomes and organizational growth, requiring strong communication and the ability to manage multiple priorities on the road.

Qualifications

• Bachelor’s degree preferred (or equivalent experience).
• Minimum of two years of healthcare or related sales experience.
• Proficiency with Microsoft Office and CRM systems.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Develop and maintain strong relationships with referral partners through consistent communication and service excellence.
• Grow market share and drive referral volume by coordinating with clinical and operational teams.
• Track, manage, and document sales activity across referral sources using CRM tools and achieve referral/admission goals.
