Job summary

VitalCaring Group is seeking a Care Transition Coordinator – Hospice in the New Orleans and West Bank area to connect patients, families, and partners with hospice services. This field-based role focuses on seamless care transitions and growth in referrals.

The position blends healthcare sales with care coordination, requiring strong relationship-building, strategic outreach, and independent, results-driven work across a dynamic team.

Qualifications

• Bachelor’s degree (or equivalent) in Healthcare, Business, Marketing, or related field preferred.
• Minimum of two (2) years healthcare or related sales experience.
• Strong communication, relationship-building and influencing skills.
• Ability to manage multiple priorities in a fast-paced, field-based environment.
• Proficiency with Microsoft Office and CRM systems.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Build and maintain strong relationships with referral partners through consistent communication and service excellence.
• Develop and execute strategies to grow market share and drive referral volume.
• Track, manage, and document sales activity across referral sources using CRM tools.
• Achieve established referral and admission goals.
