VitalCaring Group is seeking a Care Transition Coordinator – Hospice to connect patients, families, and clinicians for seamless transitions. The role blends healthcare sales, coordination, and community partnerships to support growth and patient access to hospice services.

The ideal candidate has a healthcare sales background, strong communication skills, and drives results in a field-based setting. Travel within Texas is required; a valid driver’s license is essential.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field or equivalent experience.
• At least 2 years healthcare or related sales experience.
• Strong communication and relationship-building skills.
• Ability to manage multiple priorities in a fast-paced field-based setting.
• Proficiency with Microsoft Office and CRM systems.
• Ability to work independently and drive results.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing
• Build and maintain strong relationships with referral partners through consistent communication and service excellence
• Develop and execute strategies to grow market share and drive referral volume
• Track, manage, and document sales activity across referral sources using CRM tools
• Achieve established referral and admission goals
