Job summary

You will serve as a local resource, often traveling to homes, shelters, and provider sites to support benefits access and care coordination. Responsibilities include outreach, scheduling, transportation coordination, and linkage to social services, with 50-80% local travel as required by state contracts.

Qualifications

• 1 year health care experience with underserved populations or equivalent.
• CHW certification may be required for certain states.
• Valid driver's license, reliable transportation, and auto insurance for travel.
• Ability to multi-task while speaking with members.
• Excellent customer service skills.
• Organizational and time-management skills.
• Willingness to learn other programs and software systems.
• Excellent verbal and written communication.
• Microsoft Office proficiency.

Responsibilities

• Advocate for members and support management of health care needs.
• Work with health care services team performing non-clinical duties in the field.
• Help navigate benefits, schedule appointments, arrange transportation, and follow up on missed visits.
• Assist members in accessing social services and community resources.
• Provide outreach to locate disconnected members with special needs.
• Conduct research to locate hard-to-reach members using databases and field outreach.
• Participate in ongoing or project-based member outreach efforts.
• Guide members to maintain Medicaid eligibility and other financial resources.
