Molina Healthcare seeks a community-based Member Advocate to support vulnerable members in managing health care needs. You will serve as a local resource, helping navigate plans, arrange appointments, and access social services in the community.

You will travel 50-80% locally, meeting members in homes, shelters, and provider offices, with emphasis on customer service, organization, and communication skills to empower members and maximize benefits.

Qualifications

• At least 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required for certain states depending on contractual requirements.
• Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job-related travel.

Responsabilités

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with the health care services team by providing non-clinical paraprofessional duties in the field (home visits, shelters, offices).
• Empowers members by helping them navigate and maximize their health plan benefits (scheduling, transportation, prescriptions, follow-ups).
• Provides outreach to access social services such as housing, food, employment, etc.
