Job summary

Molina Healthcare is seeking a Community Health Worker in Columbus, OH to support local member advocacy and link members to community resources. You will meet with members in homes, shelters, and provider offices to assist with health plan benefits, transportation, appointments, and social services.

You will travel 50-80% locally, engage with underserved populations, document interactions, and help maintain Medicaid eligibility and access to care.

Qualifications

• At least 1 year of health care experience in underserved populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver’s license and reliable transportation for travel.
• Ability to multi-task while speaking with members.
• Excellent customer service and communication skills.
• Organizational and time-management skills.
• Willingness to learn additional programs and software.

Responsibilities

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with health care services team providing non-clinical paraprofessional duties in the field.
• Helps members navigate and maximize health plan benefits (appointments, transportation, medications).
• Assists members in accessing social services (housing, food, employment).
• Provides outreach to locate disconnected members with special needs.
• Performs location outreach including home visits, shelters, provider offices, etc.
