Qualifications

• Minimum 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required depending on state/contract.
• Valid driver's license, reliable vehicle, and adequate auto insurance for travel.
• Ability to multi-task applications while speaking with members.
• Excellent customer service and communication skills.
• Strong organizational and time-management abilities.
• Willingness to learn other lines of business and software systems.
• Proficient in Microsoft Office.

Responsibilities

• Engage with members as an advocate and resource to support health care needs.
• Provide non-clinical paraprofessional duties in the field with home visits or in shelters.
• Help members navigate benefits, schedule appointments, and arrange transportation.
• Assist members in accessing social services and community resources.
• Outreach to locate and support disconnected members with special needs.
• Conduct research to locate hard-to-reach members using internal data and community resources.
• Participate in outreach activities, telephonic and face-to-face as needed.
• Guide members to maintain Medicaid eligibility and financial resources.
