Job summary

Molina Healthcare, Inc. is seeking a Community Health Worker to support community-based member advocacy and act as a local resource for vulnerable members.

The role includes meeting with members in homes, nursing homes, shelters, and provider offices, and guiding them to maximize their health plan benefits and social services. The position requires 1 year of health care experience, CHW certification may be required for some states, and a valid driver's license for travel.

Qualifications

• At least 1 year of health care experience, preferably with underserved populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job-related travel.

Responsibilities

• Advocate for members and act as a local resource to support health care needs.
• Meet with members in homes, nursing homes, shelters, provider offices, etc.
• Help members navigate Medicaid and other financial resources.
• Assist members in accessing social services and community resources.
• Provide outreach to locate disconnected members and support special needs groups.
• Conduct outreach and research to locate hard-to-reach members.
