Job summary

UnitedHealth Group in Charlotte, NC is hiring a Field Based Community Health Worker (CHW) to engage health plan members in their homes and communities, improve access to care, and help members utilize benefits. This is a full-time, 40-hour-per-week position conducted primarily in the field, with a home-based office when not meeting members.

The role requires a high school diploma, 3+ years in community health, field experience, and a valid driver's license.

Qualifications

• High School Diploma / GED required.
• 3+ years of experience in a community health setting or outreach.
• 2+ years of experience working with diverse multi-ethnic and socioeconomic populations.
• 1+ year of field-based experience.
• Proficient with Windows and Excel.

Responsibilities

• Engage members at homes or communities to educate on programs and benefits.
• Coordinate and schedule clinical visits to ensure timely access to care.
• Build trust with members and partners and promote engagement with providers.
• Document member interactions in the EMR and plan efficient routes.
• Assist with obstacle removal to improve utilization of benefits.
