This 40-hour-per-week field-based position includes a home-based office with potential remote work when not in the field, located in Montgomery, AL.

Qualifications

• 2+ years of field-based experience.
• 2+ years of patient interaction in the healthcare field.
• 1+ years of experience with knowledge of the resources available, culture, and values in the community.

Responsibilities

• Assess, plan, and implement care strategies individualized by patient.
• Utilize resources to establish safe, effective case management.
• Locate and engage members in the community.
• Collaborate with patient, family, and healthcare providers.
• Identify and initiate referrals for social service programs and resources.
• Communicate health information to stakeholders for coordinated care.
