Qualifications

• High School Diploma/GED required.
• 2+ years of field-based experience.
• 2+ years of patient interaction in healthcare.
• 1+ years knowledge of community resources.

Responsibilities

• Assess, plan, and implement individualized care strategies.
• Engage with community members and social service referrals.
• Coordinate with patients, families and providers to improve outcomes.
• Document interactions and referrals.
• Travel within designated area as needed.
