Job summary

Community Health Worker position at Community Care of North Carolina is a non-clinician role supporting the primary care team by providing comprehensive, coordinated community health interventions. The CHW helps with outreach, health risk screenings, referrals to Care Management, and ensuring access to resources across communities.

Based in Asheboro, NC, this primarily remote role requires travel within the region and/or the state.

Qualifications

• High school diploma or GED required.
• 2-4 years minimum experience in a human services or health care setting.
• 2- or 4-year degree in health-related field preferred.
• Bilingual in English and Spanish preferred.
• Valid driver's license with current auto liability insurance.

Responsibilities

• Abide by department guidelines, policies, and HIPAA regulations.
• Participate in community outreach and care management referrals.
• Verify eligibility and demographic information.
• Assist with care coordination and access to resources.
• Travel within the region and/or state.
