Qualifications

• Must reside within commutable distance to Montgomery, AL.
• 2+ years of field-based experience.
• 2+ years of patient interaction in healthcare.
• 1+ years of knowledge about community resources and values
• Access to a designated quiet home workspace with PHI security.
• Proficiency with MS Word, Excel and Outlook.
• Ability to travel locally ~75% with Alabama driver’s license and insurance.

Responsibilities

• Assess, plan, and implement individualized care strategies.
• Use community resources to create safe case management plans.
• Engage members in the community and coordinate with healthcare providers.
• Initiate referrals for social services including financial and state resources.
• Communicate health information to stakeholders to ensure timely coordinated care.
• Document interactions and referrals and support utilization benchmarks.
