Qualifications

• Proficient in Windows and Excel.
• Must have reliable home workspace and internet.
• Driver's license and reliable transportation.
• Within commutable distance to Charlotte, NC.

Responsibilities

• Engage members at homes, senior living and care facilities.
• Educate members on programs and benefits.
• Coordinate scheduling for clinical visits to ensure timely care.
• Build trust with members and health care providers.
• Help members overcome barriers to usage of benefits.
• Document engagement attempts in the EMR and maintain privacy.
• Plan routes for efficient scheduling and travel time.
