Job summary

UnitedHealth Group is seeking a Field Based Community Health Worker to engage members in Milwaukee and surrounding Wisconsin communities. You will educate about plan benefits, coordinate visits, and help members access care in a field-based and home-based setting.

This full-time role requires field experience, reliability, and the ability to travel within a designated area. A dedicated work-from-home setup and flexible hours support effective member outreach.

Qualifications

• High School Diploma/GED.
• 3+ years of experience in community health or outreach.
• 2+ years working with diverse multi-ethnic populations.
• 1+ years of field-based experience.
• Intermediate level of Windows experience with Excel.
• Must reside within commutable distance to Milwaukee, WI.
• Dedicated work area with privacy and data protection.
• Active and unrestricted driver’s license with reliable transportation.
• Must have access to high-speed internet at home.
• Plan routes for efficient visits and travel time.

Responsibilities

• Engage members at homes, senior living facilities, or community sites.
• Provide education on programs and benefits.
• Schedule clinical visits to ensure timely access to care.
• Build trust with members and coordinate with providers.
• Identify and remove barriers to using benefits and services.
• Address challenges to improve member experience and engagement.
• Complete assessments and identify health plan needs.
• Document engagement attempts in the EMR and ensure accuracy.
• Plan routes to optimize field time and travel distance.
