Job summary

Optum is seeking a Field Based Community Health Worker to engage health plan members in home and community settings, helping them use benefits and access care. The CHW will meet members where they are, educate them on programs, and coordinate clinical visits while representing Optum professionally.

This full-time role requires strong communication, organization, and the ability to navigate diverse populations in Oklahoma City and surrounding areas.

Qualifications

• High School Diploma/GED required.
• 3+ years in community health or outreach.
• 2+ years working with diverse populations.
• 1+ years in field-based work.
• Proficient with Windows and Microsoft Excel.
• Reliable transportation and valid US driver's license.

Responsibilities

• Engage members in homes, senior living facilities, and communities.
• Provide education on programs and benefit plan offerings.
• Schedule and coordinate clinical visits for timely access to care.
• Build trust with Optum healthcare providers and members.
• Assist members in utilizing health plan benefits and resources.
• Document engagement attempts in EMR and plan routes efficiently.
