The CHW will schedule visits, build trust with members and collaborate with care providers to improve access and utilization of benefits. A private work area is required and some telecommuting is allowed for SC residents.

Qualifications

• Diploma or GED required; CHW certification preferred.
• 3+ years in community health or outreach.
• 2+ years working with diverse populations.
• 1+ years field-based experience.
• Proficient Windows navigation; Excel skills.
• Private work area ensuring information privacy.

Responsibilities

• Engage members at residential homes, senior living facilities or in communities.
• Educate members on programs and benefit plan offerings.
• Schedule clinical visits to ensure timely access to care.
• Build trust with members and coordinate with providers.
