Job summary

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required for certain states.
• Valid driver's license, reliable transportation for travel.
• Ability to multi-task while assisting members.
• Excellent customer service and organizational skills.
• Proficiency in Microsoft Office tools.

Responsibilities

• Engage with members as advocate and resource for health care needs.
• Provide non-clinical paraprofessional support in the field (homes, shelters, offices).
• Help members navigate health plan benefits and schedule appointments.
• Assist with access to social services and community resources.
• Reach out to locate disconnected members with special needs.
• Travel locally 50–80% as required.
• Guide members to maintain Medicaid eligibility and financial resources.
• Participate in outreach activities (telephonic or in-person).
