Qualifications

• 1 year health care experience, preferably with underserved populations.
• Valid and unrestricted driver's license and reliable transportation for travel.
• Excellent customer service and communication skills.
• Ability to multi-task while speaking with members.
• Willingness to learn other programs and software systems.

Responsibilities

• Advocate for members and help manage health care needs.
• Meet members in homes, nursing facilities, shelters, or offices for non-clinical support.
• Assist with navigating benefits, scheduling, transportation, and prescriptions.
• Help access social services and community resources.
• Travel locally 50–80% as required by state/contractual needs.
