Molina Healthcare, Inc. is seeking a Community Health Worker to support community-based member advocacy and navigate health plan benefits. This role involves in-home or community visits, assisting members with scheduling, transportation, and accessing social services.

Ideal candidates have health care exposure, strong communication, and willingness to travel locally up to 80%. CHW certification may be required depending on state contracts.

Qualifications

• 1 year of health care experience in underserved populations preferred.
• CHW certification may be required in certain states.
• Strong customer service and communication skills.

Responsabilités

• Advocate for and assist members in managing health care needs in the community.
• Collaborate with health care services team on non-clinical duties in field locations.
• Help members maximize Medicaid benefits and arrange transportation for visits.
• Assist with connecting members to social services like housing, food, and employment.
• Reach out to hard-to-reach or disconnected members.
• Conduct outreach to locate members via databases, providers, or community sites.
• Participate in outreach activities, telephonically or in person.
• Guide members to maintain eligibility and access financial resources.
