You will assess needs, facilitate referrals, and advocate for patients while navigating STAR+Plus program requirements and maintaining on-site responsibilities at the El Paso location.

Qualifications

• High School Diploma or GED required.
• Direct experience with ABD/SSI population in 3 of the last 5 years.
• 4+ years of experience in community health setting.
• 1+ years of experience with MS Word, Excel, Outlook for data entry and word processing.
• Experience in long-term care, home health, hospice, public health or assisted living.
• 1+ years of experience with electronic charting.
• Knowledge of community resources, culture, and values.
• Bilingual English/Spanish.
• On-site work requirement at 1145 Westmoreland Dr, El Paso, TX 79925.
• Residence in El Paso, TX.

Responsibilities

• Assess, plan, and implement individualized care strategies.
• Coordinate with resources to create safe, effective care plans.
• Collaborate with member, family, and healthcare providers.
• Identify and initiate referrals for social services.
• Manage care plan and serve as single point of contact.
