Job summary

CVS Health is seeking a Field Health Worker to serve as a bridge between members and healthcare providers in LA Region 1. The role requires extensive in-home outreach, culturally sensitive care, and ongoing education to improve health outcomes.

The position emphasizes travel, coordination with social services, and documentation in electronic health records, with a focus on reducing hospitalizations and barriers to care.

Qualifications

• Ability to travel 70% of the time with reliable transportation.
• 3-5 years' relevant work experience.
• 1-2 years' experience providing clinical support services.
• 1 year of community outreach experience and knowledge of LA resources.
• 1 year of experience working with clients with clinical needs (physical and/or behavioral health).
• Proficiency with MS Office and EHR systems.

Responsibilities

• Conduct home visits to reinforce care plans, adjust tasks to fit member culture, language, and religion, improve access to care and reduce hospitalizations.
• Educate healthcare professionals on social determinants of health affecting member care and treatment plans.
• Build and maintain relationships with members, providers, nurse case managers, agency staff, and supervisors from diverse backgrounds.
• Work to reduce cultural and socio-economic barriers between clients and institutions.
• Assist with care plan implementation and member education during in-home visits and coordinate linkages to social services.
• Enter and maintain member records in EHR, compile reports, and complete program documentation.
