Job summary

Taleo is seeking a Field Based Community Health Worker to identify and initiate referrals for social services, support members with home and social resources, and educate on resources in the El Paso area.

This full-time role focuses on addressing social determinants of health, coordinating with care teams, and ensuring timely, quality care for Medicaid members in the StarPlus program. Bilingual English/Spanish preferred and on-site at 1145 Westmoreland Dr., El Paso.

Qualifications

• High School Diploma or GED is required.
• Direct experience with ABD/SSI population preferred.
• Experience in community health settings is required.
• Bilingual English/Spanish is preferred.

Responsibilities

• Identify and initiate referrals for social service programs.
• Coordinate care plans with members, families, and providers.
• Manage the care plan and act as the single point of contact.
• Advocate for patients and families to ensure needs are met.
• Maintain knowledge of StarPlus contract requirements and regulatory needs.
