Qualifications

• High School Diploma or GED is required.
• Minimum 2+ years of field-based healthcare experience.
• 2+ years of direct patient interaction in healthcare.
• Ability to travel locally ~75% with reliable transportation.
• Must reside within commutable distance to Montgomery, AL.

Responsibilities

• Assess, plan, and implement care strategies individualized for each patient.
• Utilize community resources to create safe, effective care plans.
• Locate and engage members in the community and coordinate with families and providers.
• Identify and initiate referrals for social services (financial, psychosocial, community, state).
• Communicate health information to stakeholders ensuring coordinated, timely care.
