Job summary

UnitedHealth Group in El Paso, TX is seeking a Field Based Community Health Worker to connect Medicaid members with home and social resources and education. The role focuses on addressing social determinants of health such as transportation, housing, and food access, and coordinates care with a multidisciplinary team.

Responsibilities include assessing member needs, developing individualized care plans, initiating referrals to social services, and advocating for patients.

Qualifications

• High School Diploma or GED required.
• Direct experience with ABD/SSI populations in 3 of 5 years.
• 4+ years in community health settings.
• 1+ year experience with Word, Excel, Outlook for data entry.
• Experience in long-term care, home health, hospice, or related.
• 1+ year experience with electronic charting.
• Knowledge of local community resources and culture.
• Bilingual English/Spanish required; on-site in El Paso.
• Ability to meet the posted schedule.

Responsibilities

• Assess, plan, and implement care strategies for each member toward appropriate care level.
• Utilize resources to establish a safe, effective care plan.
• Collaborate with member, family, and healthcare providers to develop plan.
• Identify and initiate referrals for social service programs.
• Manage care plan as single point of contact.
• Communicate health information to stakeholders to ensure coordinated care.
• Advocate for patients to reflect needs and choices.
• Understand insurance products and regulations as they apply to the health plan.
• Monitor services for timeliness and satisfaction.
• Report medically complex cases as needed.
• Provide administrative support for care coordination levels 1 and 2.
