Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy and navigation of health plan benefits in the Ohio region, including travel to member homes, shelters, and provider offices.

The role focuses on assisting vulnerable members with health care needs, coordinating resources, and maintaining Medicaid eligibility, with substantial local travel required.

Competenze

• At least 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver’s license and reliable transportation.

Mansioni

• Engages with members as an advocate and resource to support health care needs.
• Collaborates with health care services team providing non-clinical paraprofessional duties in the field.
• Helps members navigate health plan benefits, schedule appointments, arrange transportation, and follow up.
• Assists members in accessing social services and community resources.
• Outreach to locate/disconnected members with special needs.
• conducts research to locate hard-to-reach members and travels to last known addresses or shelters.
• Participates in ongoing or project-based member outreach activities (telephonic/face-to-face).
• Guides members to maintain Medicaid eligibility and other financial resources.
• 50-80% local travel may be required.
