Job summary

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required depending on contract/state.
• Valid driver’s license and reliable transportation for travel requirements.
• Strong multitasking and communication skills in person and over the phone.
• Proficiency in Microsoft Office or applicable software.

Responsibilities

• Advocate for members and help manage health care needs locally.
• Meet with members in homes, shelters, and provider offices.
• Assist with navigating benefits and scheduling appointments.
• Connect members with social services and community resources.
• Travel 50–80% locally as required by state contracts.
• Support outreach to disconnected members and maintain Medicaid eligibility guidance.
