CVS Health is seeking a Case Management Coordinator in Illinois to assess, plan, implement, and coordinate case management activities for members to support overall wellness. The role requires travel 50-75% and involves evaluating medical needs, benefit plan eligibility, and coordinating with Aetna programs as needed.

The position emphasizes clinical judgment, holistic assessment of co-morbid conditions, and collaboration with supervisors to overcome barriers and ensure effective care planning.

Competenze

• Bachelor's degree or non-licensed master level clinician required, preferable with behavioral health or human services
• Minimum of 2 years of experience in case management, behavioral health, social services, or related field
• Willing and able to travel 50-75% of time to meet with members face to face in surrounding areas
• Valid Drivers License, preferably in IL

Mansioni

• Assess and coordinate case management activities telephonically and face to face to evaluate medical needs and wellness
• Develop proactive plans to improve short- and long-term outcomes
• Evaluate member needs and benefit plan eligibility and facilitate transitions to Aetna programs
• Apply clinical judgment to reduce risk factors and barriers and address complex indicators
• Review prior claims and assess work capacity and functional restrictions
• Refer to clinical resources as needed and present cases at case conferences for multidisciplinary focus
