Qualifications

• Bachelor's degree in behavioral health or related field.
• 2 years of case management or discharge planning experience.
• Managed Care experience preferred.
• Ability to travel within designated geographic area.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.

Responsibilities

• Evaluate members’ needs and eligibility and recommend case resolution and care planning.
• Identify high risk factors and refer to clinical case management.
• Coordinate and monitor assigned care plan activities.
• Collaborate with Medical Directors and other programs to improve care quality.
• Escalate quality of care issues through established channels.
• Assist members with scheduling, benefits access, and education.
• Engage in team meetings and provide peer mentoring.
• Support member participation in healthcare decisions.
• Document care in compliance with regulatory guidelines.
