Job summary

Qualifications

• Two years of case management experience required.
• Proficiency with Microsoft Office and electronic health records.
• Willingness to travel up to 75% and have a valid Illinois driver's license.
• Reliable transportation and ability to work in Illinois St. Clair County area.

Responsibilities

• Provide care coordination, support and education to members.
• Evaluate referred members' needs/eligibility and plan care.
• Coordinate and monitor care plan activities and progress.
• Collaborate with case managers, supervisors and Medical Directors to improve outcomes.
• Document care processes in compliance with policies and regulatory guidelines.
