CVS Health Corporation is seeking a Case Management Coordinator to join the Aetna team in Illinois. You will collaborate with members, providers, and community organizations to coordinate care and access benefits, aiming for optimal health outcomes.

We require 2 years in behavioral health or related field, residence in Illinois, and willingness to travel up to 50–75% across Southern Cook County. The role offers full-time hours and comprehensive benefits.

Qualifications

Must reside in Illinois.
Willing to travel up to 50-75% of the time in Southern Cook County and surrounding areas.
Proficient with Excel and Microsoft Word.
Effective communication, telephonic and organization skills.
2 years' experience in behavioral health or related field.
Ability to work independently and as part of a multi-disciplinary team.

Responsabilités

Evaluate referred members' needs and eligibility and determine a course of action.
Identify high risk factors and arrange appropriate referrals.
Coordinate and monitor care plan activities.
Collaborate with case managers and Medical Directors to improve care quality.
Identify and escalate quality of care issues through established channels.
Negotiate options to meet members' benefits and healthcare needs.
Engage members to promote healthy behaviors and decisions.
Document care and ensure regulatory compliance.
