Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Must be willing and able to travel 50-75% in the Metro East, Illinois area with reliable transportation.
• 2+ years of electronic documentation experience and Microsoft Office proficiency.

Responsibilities

• Evaluate Members and assess needs and eligibility, recommending care plan approaches.
• Identify high-risk factors and coordinate with clinical teams for appropriate referrals.
• Coordinate, implement, and monitor assigned care plan activities and progress.
• Consult with care managers and Medical Directors to overcome barriers and improve outcomes.
• Escalate quality of care issues through established channels.
• Engage members and providers to support informed healthcare decisions.
