Job summary

Humana is seeking a Care Coordinator to support Illinois HealthChoice Medicaid Managed Care in the Humana Gold Plus Integrated program. You will assess members' needs, develop personalized care plans, and connect families with resources to improve health and well‑being while navigating the healthcare system.

This is a field-based role requiring 75–90% travel to member homes and facilities; a valid IL license or equivalent certification may be required depending on eligibility.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN in Illinois with one year experience conducting comprehensive assessments and provision of formal service for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings – 75%-90% local travel.
• Assess and evaluate enrollee needs via telephonic and face-to-face assessments to coordinate services and address enrollee needs.
• Identify barriers and develop interventions to meet enrollee care goals.
• Coordinate services, monitor and evaluate care plans against member goals.
• Assist enrollees with filing grievances and appeals and provide information on MCO requirements, services, and benefits.
• Guide members toward resources and facilitate interactions with providers and other payer sources.

Education

Bachelor's degree in social sciences, social work, human services, or a related field
