Responsibilities

• Visit Medicaid members in homes or care facilities (75–90% travel).
• Assess care needs and develop personalized care plans.
• Coordinate services and monitor progress toward goals.
• Identify barriers and implement interventions.
• Assist with grievances and appeals processes.
• Provide information on MCO requirements and community resources.
• Support member navigation of the healthcare system.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted LPN in Illinois with 1 year experience

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
