Job summary

Humana is seeking a Care Coordinator for the Illinois HealthChoice Medicaid Managed Care program. You will assess members, develop personalized care plans, and connect families with resources to improve health and well-being. This field-based role reports to the Manager of Care Management.

Responsibilities include home visits, care planning, and coordinating services to reach personal health goals, with a focus on patient-centered care and advocacy.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Unrestricted LPN in Illinois with 1 year experience in assessments and elderly services.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in homes or facilities 75-90% travel.
• Assess care needs and ensure services in least restrictive setting.
• Identify and resolve barriers to care.
• Plan and implement interventions to meet needs.
• Coordinate services and monitor care plans.
• Monitor patient progress toward outcomes and update plans.
• Assess enrollee needs via telephonic and face-to-face evaluations.
• Assist enrollee with gaps in care and with grievances/appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer enrollees to community resources and guide to appropriate services.
